In consideration of being allowed to participate in any way in Discovery Lab activities, I, the parent and/or legal guardian of the minor named below agree to assume all risk of any kind of injury or damage my child, I, and any member of my Child, me and my party will obey Discovery Labs code of conduct, signs and rules at all times (displayed in the premises).

I understand and acknowledge that I am fully aware and assume the risks (included but not limited to the risk of poisoning, allergic reactions to ingredients, chemicals and materials used at Discovery Lab, the risk of any type of bodily injury, property loss or damage) of said minor child’s participation, my party's and my participation in recreational activities with Discovery Lab and by any and all experiments performed under their supervision. I recognize my responsibility to ensure that said minor participates only in those activities for which he/she has the required skills, age, qualifications and physical conditioning. I understand that Discovery Lab shall have no responsibility to pay for medical treatment and related costs if myself or said minor is injured.

I KNOWINGLY AND FREELY ASSUME ALL RISKS, BOTH KNOWN AND UNKNOWN, AND HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS, FOR MYSELF, THE ABOVE NAMED MINOR AND OUR HEIRS, DISCOVERY LAB, THEIR TRUSTEES, OFFICERS, EMPLOYEES AND VOLUNTEERS WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, or loss or damage to person or property associated with the presence or participation of the minor in my charge, my party and myself, WETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fully extent permitted by law.

PHOTO RELEASE: I hereby grant and authorize Discovery Lab Inc the right to edit, alter, copy, and make use of all photos and/or videos taken of myself and below-named minor’s to be used in and/or for promotional materials without payment or consideration. This grant of use includes but is not limited to publishing on Internet and e-mails, magazines, pamphlets, advertisements, fliers, social media, and in whatever other manner the company finds useful or for any other lawful purposes. In addition, I waive any right to inspect or approve the finished product wherein above named minor or myself appear. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless, release, and forever discharge Discovery Lab from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

COVID-19 HEALTH LIABILITY: I, the parent and/or legal guardian of the minor named below, as well as the minor, have not within the past 14 days of the scheduled event been in contact with or cared for someone who has been diagnosed with COVID-19 or suspected to have COVID-19. We have not experienced any symptoms associated with COVID-19, such as: fever, cough, sore throat or shortness of breath in the past 24 hours.

 

I, the undersigned, acknowledge that I have carefully read this release of liability and understand its contents.
